Steps

1. open the following url in a new Chrome tab:

chrome-extension://ldmmifpegigmeammaeckplhnjbbpccmm/popup/index.html?standalone=1&formName=YOUR_FORM_NAME

2. if you want to open a specific form, replace YOUR_FORM_NAME by the form name. it needs to be in camel casing (no space):

examples:
